Case Manager and Director: Mary “Chiz” Chisholm

Solely funded by donations, Chiz’s Heart Street Inc. 501 (c)(3)is a non-profit charity which provides Independent Case Management services. Chiz’s Heart Street started in 2004 in Kingston, New York and as of Sept 2021 transitioned to its current location based out of San Diego California — but responding, as best we can, to anyone who calls us for help wherever they may be.

Especially with growing homelessness, and the restrictions related to Covid, Case Management is more sorely needed but less available than ever. “Chiz” has over 30 years of wide ranging experience in serving the most marginalized and challenging populations with dignity and compassion.

 At no cost to either the client or to agencies, our case manager “Chiz” meets people where they are at on the street, human to human, in particular focusing on those struggling with chronic homelessness, mental illness or addiction.

Whatever it takes to help — from buying a pair of shoes, taking someone to an appointment, assisting with access to housing or treatment, to just sitting and talking for a while. Our focus is always on meeting that person’s unique individual needs as directly, rapidly, efficiently, and kindly as possible.
